Registration

2013 Registration Information:

• Registrations forms can be downloaded from the website or picked up at the NICK SMITH CENTRE.
  Please make your payment by cheque payable to "AMBUSC".
• New players, (players who have not previously registered with AMBUSC) please attach a photocopy of the player's "proof of age" – birth certificate, health card etc.
• For players who turn 11 years or older in 2013 please attach a "passport size" photo with name and date of birth on the back.

• Note: When dropping off your registration form, players who are 'NEW' to the club, and wish to play in the youth (u12-u19) or adult categories, please provide a actual passport size photographs with proof of age (for ALL youth, born in 2001 and earlier is mandatory on registration) when registering.

• If the youth player did register with AMBUSC at some point, but has not played on any team in the past year, a recent photo would still need to be provided.

• Registration Cost

• Please refer to the Age Classifications table to see what age group to register up.
• Family rate (3 children [u4-u18] or more): $215
• There will be $20 late fee for registrations after March 31, 2013.

• NOTE
  • If born 1994-2001, please bring passport size picture.
  • Cheques payable to AMBUSC.

If possible please print, fill in and bring the appropriate form and a cheque with you it will greatly facilitate the process. There will, however, be forms at the centre that can be filled in there.

Regular registration will be closed, as of April 15. Late registrants after this date should contact the club administrator and may be placed on a team or waiting list subject to space availability.
